Preparing for your visit
At Family Health Group,
we want you to get the most out of your visit.
We realize that your time is valuable and want
your visit to be as satisfying as possible. In
order to provide you with accurate information
and to adequately address your concerns, we ask
that you bring a few things with you to your appointment:
- Insurance card/cards
- Referral form if mandated
by plan or confirmation of same
- Relevant medical records
to include mammogram report, lab test results,
x-rays or other test results if not ordered
by the physician with whom you had appointment.
- Written list of all
medication, including strength and dosage. Include
prescriptions, over-the-counter medications,
vitamins and herb supplements.
- A past medical history
to include any chronic conditions such as heart,
thyroid, hypertension, diabetes, any surgeries
you have undergone and allergic reactions to
medications or other substances you may have
experienced.
- Make note of any questions
or concerns you might have pertaining to your
condition.
The Actual Visit
- Be specific about your
condition, including date of onset, duration
and symptoms.
- Discuss honestly any
issues that may contribute to the condition
even though they may seem awkward or personal.
- Take notes as necessary
on the information the doctor discusses with
you regarding his findings/diagnosis, general
treatment recommendations and prescribed medication
instructions. Ask for clarification of any questions
you may have concerning these.
Post-visit Period: Steps
to Achieve Recovery
- Follow the instructions
you were given on what to do or not to do.
- Take the full course
of any prescribed medication.
- Inform your physician
if your condition becomes worse or complications
develop. Also contact him/her at the very first
sign of a possible medication reaction.
- If you have questions
or concerns or you are simply unable to understand
or remember any information or instructions
you received during your visit, please call
the office and ask to have a member of the clinical
staff return your call.
- Keep all referral visits
or follow-up visits as scheduled
